Our stone chimneys tend to get something of a raw deal when it comes to maintenance. They suffer from the out of sight out of mind syndrome, when the reality is that they need even more attention than say the front of your house or your garden walls as the chimney is so much more exposed to the elements. There is also potentially much more potential for real disaster should a stone chimney collapse (and they do) than say a stone wall.
Typical issues with lack of maintenance on chimneys include cracked and missing lime pointing, erosion of masonry pieces causing a weakness of the structure, loose stone hanging out, loose chimney cans/pots, loose and out of alignment coping stones etc, etc. Your chimney may have a bad lean you are unaware of and it may be in danger of falling over.
